Minors
In order to protect children from abduction and to address concerns of runaway children, all children under the age of 16 are required to appear at the Consulate with both parents for all passport services. The only exceptions made are in cases where one parent is outside of Turkey or is physically unable to travel to the Consulate (for example, they are hospitalized or incarcerated). When the application is submitted, then an original statement of consent from the non-applying parent is required and must be notarized.
If one parent does not appear and the applying parent does not have a document to show his/her authority to apply alone and cannot demonstrate exigent/special family circumstances, then the application will not be accepted until an original notarized statement of consent is received from the non-appearing parent.
Application: It takes approximately ten days to receive your new passport. This includes the time it takes for us to transmit your passport application to the United States, receive the new passport and mail it to you. Please apply early for your new passport. To apply, please bring the following documents to the nearest Embassy or Consulate:
- Passport application form DS-11
- One passport photograph: The Embassy/Consulate will reject any photographs that do not meet the specified requirements
- Fees: The application fee for an applicant under the age of 16 is U.S. $105.00; aged 16 and over is $110.00. You may pay by U.S. cash or credit card at the Consulate.
- The minor’s most recent United States passport and a photocopy.
- The minor’s social security number.
- The minor’s original birth certificate or Consular Report of Birth Abroad (if a first-time applicant OR if the minor is under the age of 16) and a photocopy.
- Up to date form of identification and a photocopy for both parents. This may be in the form of:
- a valid passport of any country
- a valid driver's license with photograph
- a national identity card with photograph
- a valid U.S. or State government ID card or
- a U.S. military ID card.
- Name change documentation: If either parent has changed his/her name since the last passport was issued, legal name change documentation, such as a marriage certificate or other legal document is required
- Proof of the mother’s maiden name (i.e. expired passports, kimlik, etc.).
