Application Requirements:

It takes approximately ten days to receive your new passport.  This includes the time it takes for us to transmit your passport application to the U.S., receive the new passport and mail it to you. Please apply early for your new passport. To apply, please bring the following documents to the Consulate:

• Passport Application Form DS-11
• One passport photograph:  The Consulate will reject any photographs that do not meet the specified requirements
• $110 in the form of U.S. cash or credit card to be paid at the Consulate
• Most recent U.S. passport and a photocopy
• Your original birth certificate or Consular Report of Birth Abroad along with a photocopy
• Up to date form of identification along with a photocopy.  This may be in the form of a:
  • Valid passport of any country
  • Valid driver’s license with photograph
  • National ID card with photograph
  • Valid Federal or state government ID
  • Or U.S. military card
• Proof of eligibility for citizenship: A copy of the US passport of one of your parents or a copy of your mother’s visa indicating her status in the US when you were born. 
• Your social security number
• Name change documentation: If you have changed your name since your last passport was issued, you will be required to furnish name-change documentation, such as your marriage certificate or other legal document.
