Additional Passport Pages
You need to use DS-4085 in order to get additional pages in your passport. This form should only be used to add visa pages to previously issued, currently valid U.S. passports. The fee for additional pages is $82.
Please follow the instructions on the form carefully. Complete, sign, and date the application. If you want to apply by mail, please include your passport, completed form and payment in envelope and mail them to appropriate office below:

Your passport will be returned to you by UPS within 5 working days and UPS will charge 11 TL equal to $7 from you at the delivery.
